Powder Puff Football Game

Permission Slip for Powder Puff Football

Our annual Powder Puff Football Game will be held on May 23rd at 5:30. Rain or Shine!
Practices will be held during school hours, starting the week of the event.
All students who are participating in the game will be required to stay after school on the day of
the event, May 23rd.
If you would like to participate in the game the fee is $15. This will cover the participant's
t-shirt for the games. Both, fee and permission slip must be returned in an envelope and taken
to the office.

● DEADLINE: Each participant must return the permission slip & fee by 3:30 p.m.
on Wednesday, April 30th to the office.
● This is a flag football game, tackling will not be tolerated, however; there is still a
risk of injury as in any sport.

I have read the above and understand that this is a touch football game played for fun as
part of our end of year activities.

CONSENT TO PARTICIPATE AND RELEASE OF LIABILITY


I am the parent of _____________________________________________ and give my
consent to my child’s participation in the ‘25 NWMS Powder Puff Football Game.
I do not know of any health problems or conditions that would limit or restrict my child’s
participation in the games.
I understand the physical risk involved in this type of activity and will not hold the school, the
sponsoring organization, or anyone associated with this activity, directly or indirectly, responsible
for injuries to my child which might occur. I will be responsible for the cost of care required
should an injury occur.

I acknowledge that certain risks of injuries are inherent to participation in a Powder Puff Football
Game. These types of injuries may be minor or serious and may result from one’s own actions,
the actions of others, or a combination of both. In addition, I will practice good sportsmanship.
